HOW TO SUBMIT AN ENTRY

1. CV

A full resume of work experience and duties performed (no more than 2 pages). 

2. Statement of opinion

A statement of opinion on how you bring value to your organisation in no more than 500 words (to be completed within the entry form). 

Evidence and or feedback of outcomes for customers (if applicable to the award category). No more than 2 pages.

3. Feedback

4. Photo

A clear high resolution head and shoulder colour photograph of you, the candidate which is suitable for publication. 

A letter from your employer endorsing your entry. 

5. Employer
